Sports Physicals
2025-26 School Year
For JV and Varsity Sports:
Fall – Your child must have a physical dated 8/1/24 or later
Winter – Your child must have a physical dated 11/1/24 or later
Spring – Your child must have a physical dated 3/1/25 or later
For Modified Sports:
Fall – Your child must have a physical dated 9/1/24 or later
Winter – Your child must have a physical dated 11/1/24 or later
Spring – Your child must have a physical dated 3/1/25 or later
Please make sure that physical is on file with the school. You may call the Athletic office at (845) 758-2241 ext. 18800 or email Leanne Kelly, RN at lkelly@rhcsd.org (for grades 9 through 12) or Holly Grant, RN at hgrant@rhcsd.org (for grades 7 through 8) to check if the physical is on file.
Any 7th or 8th grade student planning on playing a JV or Varsity sport must have a Tanner Score recorded on the physical exam form.
3. If your child is scheduled to have a physical with your physician prior to the start of the official sports practice date, your child does not need a separate sports physical. Please make sure the NYS physical form is filled out COMPLETELY and turned into the health office before the first day of practice. 4. FOR ALL STUDENTS: If your child will be self-carrying a medication, a physician’s order stating your physician attests that your student can self-carry and self-administer the medication must be on file in the health office.
A parent/guardian must also sign this form: PARENT AND PHYSICIAN'S AUTHORIZATION FOR ADMINISTRATION OF MEDICATION IN SCHOOL AND SCHOOL ACTIVITIES
If you cannot make an appointment with your physician in a timely fashion, you can make an appointment with any of the practices listed below.

When making your appointment, tell the office it is for a sports physical for Red Hook schools, and you will not be charged. You must bring the following to your appointment:
- A health history form filled out by the parent/guardian.
- A NYS Physical Form. The form must be filled out completely and MUST indicate the student’s cardiac health history has been reviewed.
